Your electronic patient record has a ‘nominated pharmacy’ recorded. This is the pharmacy that your GP will send prescriptions to automatically, unless you ask for the prescription to be sent to a different pharmacy.

We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
