We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
If we do not have a record of a clinician requesting you to send in a photograph(s), the photograph(s) will be discarded.
If a clinician has not requested for you to send a photograph(s) but you have concerns over a mole/rash/lump/lesion etc, contact the practice directly.
